Health Services

Health services are provided by our Wellness Centre, which is staffed by two registered nurses. Our nurses provide primary care to students who become ill or injured at school and also work with our teachers to ensure that boys coping with chronic illnesses and injuries are appropriately accommodated in their learning environment. Services include:

  • Student point of care health education
  • Concussion support
  • Mental health support
  • Consultation with parents & outside professionals
  • Consultation with outside professionals
  • Staff prevalent medical condition education
  • Consultation with outside professionals

  • Student Vaccination Policy

    Crescent parents must agree to have their child vaccinated against nine designated diseases or have a valid exemption in accordance with Ontario’s Immunization of School Pupils Act (ISPA). Parents are required to submit their child’s vaccination information to Toronto Public Health. Students with out-of-date or incomplete immunization records are at risk of school suspension until records or a valid exemption form are provided.

    Crescent School is committed to providing and maintaining a safe school environment for all. Consistent with this commitment, the School strongly recommends that all students receive recent and up-to-date vaccines for the seasonal flu and any other pandemic for which a vaccine is available and recommended by Health Canada and/or the Ontario Chief Medical Officer of Health.
  • Supporting Students with Prevalent Medical Conditions (Anaphylaxis, Asthma, Diabetes and/or Epilepsy)

    In accordance with the Ontario Education Act, Policy and Program Memorandum No. 161, Crescent has policies in place to promote the safety and wellbeing of Crescent students with prevalent medical conditions. All students with anaphylaxis, asthma, diabetes and/or epilepsy are required to submit Plan of Care forms at the beginning of each school year. These forms outline condition triggers and management steps. They also seek authorization for the school to share this information with the student’s teachers, coaches, trip leaders and the school nurse as needed.

    It is imperative that the School be notified if a student’s medical health changes during the school year.

  • Allergy and Anaphylaxis Management

    Crescent School is an allergen-aware and nut-safe environment. This means there will be no foods or snacks served or brought to the school that are known to contain peanuts, peanut by-products, nuts, or traces of nuts.

    It is important for parents to notify the School and the Wellness Centre of any life-threatening allergies and that these boys carry a personal Epipen. If there are any changes to a student’s allergies during the school year, the parents should notify the School and the Wellness Centre. Parents whose sons have an anaphylactic allergy are required to submit a Plan of Care form each year.

    Anaphylactic students travelling off-campus must carry their own Epipen.

  • Concussion Management

    It is important for parents to immediately notify the Wellness Centre if their son has or is suspected to have suffered a concussion. The Wellness Centre will provide the family with a concussion package that includes a referral form to be completed by their family doctor. Our school nurse and learning support specialists will manage the return-to-school and return-to-sport/physical activity plans for boys in recovery. 

    Crescent School, in accordance with Rowan’s Law and the Education Act, has a Concussion and Head Injuries Policy. This policy outlines the steps taken to educate and prevent concussions and the role that the School, parents, and students play in this process. The policy also outlines the management procedures for a student’s safe return to school and sport/physical activity after a concussion.

  • Supporting Students Who Require a Service Animal

    In accordance with the Ontario Education Act, Policy and Program Memorandum No. 163, Crescent has a policy in place to support students requiring service animals while at school.

    A request for a student to be accompanied by a service animal in school should be made to the Executive Director of Crescent Student Services and the School Nurse. This request should include appropriate medical documentation and documentation showing the service animal’s training. Crescent Student Services will work in consultation with the family, student, and medical professionals to develop a plan to ensure that the student is accommodated appropriately while at school.
  • Student Immunization Records

    The Immunization of School Pupils Act (ISPA) requires that all children attending school have up-to-date immunization records on file at their local public health unit, unless the student is exempt in accordance with Ministry of Health regulations, and states that it is the parents’ responsibility to ensure their children’s immunization records are complete and current. Immunization records for your child can be verified and updated online at https://tph.icon.ehealthontario.ca. Students with out-of-date or incomplete immunization records are at risk of school suspension until records or a valid exemption form are provided.
